Documentation fails when it becomes so heavy that nobody can sustain it. A useful system should scale with the problem.

For a one-off issue that is resolved, extra paperwork may add nothing. When behaviour repeats, a short dated update can be more useful than rewriting the entire first incident. When the first report receives no meaningful response, a concise written follow-up creates the next part of the history.

The record should grow because the risk, repetition or process grew — not simply because something irritating happened in class.

That is the difference between admin and evidence: one consumes time; the other preserves the facts you may later need.
